Injury related yes, personal lifestyle related no.

Anyone that actually watches a united game and doesn't just follow what opposition fans or the media say would see that he doesn't stop running and will defend and attack all game.

Personal attitude has sucked at times. This is the only reason that I wouldn't mind if we sold him. Nothing to do with fitness.
