if it comes of a group of hands then benefit of doubt goes to the defender and its a kick out.
if a defender touches it through then ball up - it's not that much of a punishment like a free kick to the attackin team would be.
its quite simple. whenever the ball goes through goal line and was touched by a defender last, it is a ball up/throw in. Whenever a forward touches it last it's a kick out.
