You're definitely more observant than some of the randos who have asked me if my twins were identical (often without even bothering to look at them). For a sizeable number of people, the question about identical seems to come out automatically whenever they hear the word twin.

There is a very rare circumstance that allows for this. Following the monozygotic split, one of the twins can lose the Y Chromosome and have X Chromosome monosomy (Turner Syndrome); their karyotype only has X for the sex chromosomes, rather than XX, but they are genetically female. It may cause some issues with physical stature and fertility in the female twin.
