It's usually the worst drugs (ie the least actual fun, and often the most addictive) that end up being used by addicts to self medicate.
So actually in a very real way some of what you call hedonism is a very real result of society failing to provide almost exactly the same thing (medical treatment and some forms of welfare) in the first place that you are saying shouldn't be provided.
