The car door wasn't supposed to fly toward the film crew like that and wasn't far from hitting them all. Turns out you're supposed to chain all the parts to the chassis when exploding a car. They left it in the movie.
 
The first cut of First Blood was 3 hours long and was so awful Stallone wanted to buy the negative and destroy the film. He later convinced the director/producers to cut down his role dramatically and keep all the secondary character arcs. The intention from Stallones point of view was to minimize the damage but it ended up making the film a lot better and even became the blue print for action films of the 80s and 90s. Not only that it saved Stallones acting career, having no major box office successes since Rocky.

While Kurt Russell is a widely praised actor who has starred in many memorable films, he's had a strange run as box office poison. Used Cars, The Thing, Big Trouble in Little China, Deathproof, Tango and Cash, Vanilla Sky, Breakdown, and Executive Decision didn't make as much money as predicted.

The reason why one of the worst movies of all time in Baby Geniuses got a sequel is the original was a financial success at the box office !

To get the funding needed to make Barfly, director Barbet Schroeder walked into the Cannon Group office with an electronic saw and threatened to cut off his limbs. The threat worked.

Burt Reynolds turned down a role in Terms of Endearment which had been written with him in mind.

He decided to do City Heat instead, which flopped.

Jack Nicholson took the TOE role and won Best Actor Oscar.

Reynolds career tapered off after that

John Travolta turned down An Officer and a Gentleman and Days of Heaven which were both taken by Richard Gere

Members of the Irish Army Reserves were used as extras in "Braveheart". Some of the scenes had to be re-shot when it was noticed that some of the soldiers in the background were wearing wrist-watches and sunglasses.
Jack Hawkins as a Roman Tribune and naval commander in BenHur it is said , his pale skin where his watch band had been, can be seen in the sea battle scene.
A Cgrade swords and chariots Hercules movie made in the sixties an airoplane jet stream can be seen in the sky.

John Wayne was considered for the role of Dirty Harry which was to be set in Seattle.

But Clint Eastwood got the job and the location changed to SF to be closer to his home.

Wayne later made McQ, a movie about a ma verick cop set in Seattle which had many of the hallmarks of a Dirty Harry vehicle
 
James Stewart and Jesse White played two of the major characters in the 1950 film Harvey (about a man who he alone can see a 6ft tall rabbit). They both reprised their roles from the original 1940s Broadway production, and would again later in the 1970 Broadway revival, as well as the 1972 TV remake.

Stewart would later remark that he finally nailed his role in the telemovie.
 
Arnold Schwarzenegger was banging Brigitte Neilson while they filmed Red Sonya. Arnie was duped into thinking his role was a minor one to fulfill his 3 picture conan deal but realised it wasn't when the Red Sonya poster had him displayed over Neilson. Sly Stallone also banged Brigitte Neilson and made reference to their splitting up in Tango and Cash when he says to Kurt Russell 'I hate Danish'.

James Spader and Jaye Davidson only acted in Stargate for the paycheck. Initially Spader was going to refuse but couldn't help admiring Roland Emmerich's passion for the project after meeting with him. Davidson demanded $1 million thinking they'll turn him down and that'll be the end of it, only to find they accepted.

Bill Paxton is the only actor to be killed by a Terminator, Alien, and Predator (so far).

Many of the crappy direct to dvd sequels to Hellraiser use old acquired scripts that are adapted to include Pinhead and his Pandora's Box.
