Should have been locked away forever before this incident started given his history. Gargasoulas is 100% to blame for what happened but the police and courts could have prevented it.

Gargasoulas' criminal history records 253 charges dating back to his childhood, including numerous offences for dishonesty, violence, escaping from custody, weapons offences, assaulting police and drug use. He is also listed as a perpetrator in 10 family violence incidents and has a history of breaching intervention orders, the report says.

Mad Dog that should've been " taken out behind the shed " years ago.

A cluster* of mistakes.

One of Victoria's top cops says he cannot comprehend why the officers who desperately hunted James Gargasoulas didn't interview the convicted killer's girlfriend, who they had in custody, until after he mowed down pedestrians in Melbourne's Bourke Street Mall.

I suspect the cops thought she lacked credibility given she was his girlfriend.

The Bourke Street killer's girlfriend sobbed in horror, "Oh my God, Oh my God," on learning he had run people down, as he had warned her he would earlier that day.
Harrowing audio of the police interview with Akiir Muo was played at an inquest into the tragedy on Tuesday.
Ms Muo was taken hostage by James Gargasoulas after she witnessed him stabbing his brother early on January 20, 2017.
He warned her if police found him, he would "run everyone down in the city".
As police followed Gargasoulas, he pushed Ms Muo from his stolen car near the West Gate Bridge before leading officers on a chase into the city.
Ms Muo was then taken to a St Kilda police station where she was kept in an interview room for hours and not spoken to until after the Bourke Street Mall rampage.
