The flaw in this plan is that doctor claims are can't be confirmed and therefore aren't trustworthy. The only way I can see this working is if the lovers out themselves as they can affirm each other. They wax the amulet between each other and doctor stays hidden to protect the one who doesn't have it. That would require outing two roles though.

But if the doctor gets killed, then we lose both of them straight away.
