Yes but not in the hands of umpires, should be a separate selected panel at the game who have the opportunity to review the incident before handing down the verdict.

Yes but it needs to be judged by a panel of ex players after the game and it's only effective for the following game. If it's bad enough red cards can be given for multiple future games in a row.
