The old email blind (bcc) copy snafu.

They really should stop using bog standard email for these types of catastrophically confidential communications.

Way too easy for a human to cc: instead of bcc:

A communication tool/account that does not allow cc: should be used for these types of communications.
Every one of the communications should be prepared, and then quality controlled, double and triple checked before anyone can even get anywhere having access to hitting the send button.

VICPOL should go talk to security experts on what others do to prevent accidental cc: on highly sensitive communications.

Victoria Police is providing regular updates to a closed list of 104 serving and 55 veteran police members involved in the current royal commission. We do this because we are committed to supporting them through this difficult period. Our standard practice has been to blind copy recipients on these email updates, but regrettably this did not occur on an update sent out earlier this week, and the email addresses were visible to all recipients. When the mistake was identified the email was recalled immediately.Victoria Police has called or attempted to call every person on that list to apologise for the error and we have also changed our process to ensure it does not occur again. Whilst the error is regrettable, and we apologise unreservedly to all affected, we note that every recipient is a veteran or serving police officer, who is well aware of their obligations to protect the security of information.

Someone who would have been about 24yo at the time has been arrested for Richard Mladenich's murder .

A man has been arrested over the execution-style shooting death of Melbourne gangland identity Richard Mladenich nearly 20 years ago.
Police on Wednesday said they arrested a 44-year-old Brunswick West man who will be questioned over the murder as well as other crimes.

Mladenich, 37, was shot in the head at St Kilda's Esquire Motel on May 16, 2000. He had previously worked as a bodyguard for crime boss Carl Williams.
