All Lords have to start off as mere altar boys. In turn, see Vader's assistants and whatnot, when he had commanded respect and earned fear, all became his errand boys and girls too. In the EU, eventually Palpatine was always plotting and pitting other wannabe Lords against Vader anyway, suspicious Vader was going to eventually overthrow him (as per the Sith "rule of two" is want to do), and also to continually test his mettle. And Vader eventually wanted to overthrow Palpatine and rule the galaxy with his own chosen underling, long before Luke was discovered.

In SW you see the s**t rolling downhill. Palpatine condescending and bitchy with Vader, ordering him around. Vader steaming under his helmet. Goes and takes it out on those beneath him in command -- how many admirals/etc he killed for failing simple tasks, how many others he bossed around with even more condescending words. Vader so agitated by Palpatine, he butchered thousands of beings across the galaxy. Living with Palpatine would've been impossible.
