I must admit I've lost track of it in recent years but there was certainly a time when the top three and the bottom one were in the play-offs. I seem to remember Man City missing out despite coming first? But regardless of who is in the play-offs, someone has now been forced to win the play-offs rather than automatically being promoted - say the third team.
