Confused Jetstar pilots forgot to lower the wheels and had to abort a landing in Singapore just 150 metres above the ground, after the captain became distracted by his mobile phone, an investigation has found.

...

The pair had lost their "situational awareness", leading to poor decision-making and hampered communications, investigators found.

Geez, you'd like to think these guys are switched on while the lives of dozens if not hundreds of passengers are in their hands, especially during something so obviously unimportant as landing.

The fatigue possibility needs to be explored though.
 
So...the flight crew requests that passengers switch their mobile phones off and the flight crew didn't switch their mobile phones off. Nice.

This kind of thing happens all the time at Tiger. No biggie.

Heard a story that after a major delay and several flights taking of near each other, someone got on a plane in Melbourne trying to get to Tasmania, only to realise that he flying to Adelaide when they made the "we will be landing soon" call.
