It generally means individual aspects are all on point to bring about an overall good result.
eg. life, someone has got a good career, good relationship, good personality etc, to give an overall good result in terms of how their life is.
eg. looks, where someone has say good skin, good hair, nice face, fit body etc, to give an overall good result in terms of attraction.
Can apply the same principle to a passage of play in sports.

Someone being 'well put together' simply means:
Chic - Physically attractive, well proportioned
Bloke - Well built, muscular
I've never heard of it used in terms of someones' career, relationships, personality, morals or sense of fair play.

^I didn't really interpret the saying for anything specific, those were just two examples i created on the spot.

Yeah, that's the only way I've heard it too. You might use it while describing an attractive person without going in to too much detail or being particularly rude.
