We've had one or two done at work, can't remember the exact amount but it was relatively expensive. We also had to send it off to Microsoft who took 3 weeks to get it back to us. I'm sure there are 3rd party mobs that'll do it cheaper, but who knows what the quality would be like.
Screens for the Pro 3 are about $272.30 to buy, i had to order one for a customer yesterday and charge around $350 to replace it. They're not that difficult to replace the main thing you wan't to be carful of is cleaning out any hidden bits of glass for when you're putting the screen down, depending on which one you have obviously the prices vary
