I guess in real time the umpire sees Crouch dive under his opponents feet to get the ball and I presume the opponent tumbles over him. I don't think the intention of the rule or its interpretation was to stop Crouch going for the ball in the manner he did, but rather Goodes' method of sliding in feet first, fully protecting himself but with no regard to damage caused.
 
the other one that really gets me...

non-officiating umpires SHOULD NOT under any circumstance be allowed to overrule the guy standing 10m away.

well this is the issue, the hesitation to play the ball out has been good, I agree. The problem lies in the fact that every time someone gets pinged it's like 5 similar ones have already been let go.

there's too much grey, so too much inconsistency and it desperately needs fixing.

The crowd are calling deliberate when the ruck man punches the ball 30 m. There is quite a lot of room for error when punching the ball. Don't know if that's been paid yet but there is a chance and it's wrong.
