Just reformat, it's easy and relatively quick. It can be difficult to remove these sort of things completely.
 
Different Antivirus tools are better against certain things.

Try using Avast, Microsoft Security Essentials or AVG and see if they pick it up.

I know this virus like the back of my hand.

Definite case of smitfraud.

You have to routes to choose from:

(1) The easier route: reformat.

(2) The harder route: run through this - http://www.pchell.com/support/smitfraud.shtml. You'll likely need assistance from the spybot forums as well.

This is up there in annoyance with the sasser worm.

Had this one before Xmas last year. The one I had made it so you couldnt ctrl+alt+del or do anything to stop it, so you had to get in real quick and close down the files as they loaded when you started up.

Took me a few days to get rid of it completely, but the way I got rid of it was with MalwareBytes and ComboFix. What ever MWB didnt get, Combo Fix cleaned the rest up.
 
The first time I ran Malware Bytes it said it found 47 infected files, deleted them and restarted my computer and all was well.

Then a few hours later it came back. I ran UltimateAnti-Spyware or whatever it's called and it said it was clean, I then ran MalwareBytes and it found 17 more infected files, restarted my computer and it was gone again.

17-18 hours later it's still gone, so hopefully it doesn't come back.
What AV product were you running and what are you running now?

To be honest you are better off reformatting the HD. These ****ers are embedded in the registry and unless you know what you are doing when deleting reg keys you will destroy your HD anyway. Malware like this holds the Windows session open so you can try doing a scan in safe mode to prevent some Windows services starting (that the malware is using) however chances are that it has infected them anyway.

Do a reformat.
