Makes no logical sense but we do things all the time driven by impulse.
I never ask if people are having kids for two reasons:
1) I'm sure there's enough other people asking them
2) They may be trying and failing to get pregnant (for a lot of possible different reasons- none of which are my business)

I don't think asking someone if they would like to have/are planning to have kids is inherently offensive. Sure there's a chance you will ask a couple who are undergoing IVF and struggling or something like that and they might get upset but you can't spend your whole life walking on eggshells. You can ask someone 'are you seeing anyone?' and find out their partner was run over by a bus the day before. Doesn't make you a bad person for asking.
What I have noticed is the tone of questions like that changes with who is asking. 'So when are you having kids?' is not the same as just asking someone if they would like to have them. Same as 'When are you getting engaged?'. I have friends that have been together 5-10 years, live together etc. but have no desire to get married or have kids. That's their prerogative. I imagine they would get the shits with people asking why that is as though there is something wrong with them.

Otherwise you can't ask anything because there may be a situation someone is not happy to respond to.
For example 'did you go away on the long weekend?' - 'no I don't go away on log weekends since we had a bad car accident one long weekend'........

Its not inherently offensive but its a subject that in this day and age everyone knows can be upsetting.
Its not necesarily an offensive question but you probably shouldnt ask people you barely know (especially if youre coming off as "why dont you have kids yet" which most people do).
