It's a weight that is just water (it has gone white from being jostled around). The theory is that the unreliable distribution puts pressure on little muscle groups that are difficult to target in traditional, stationary weightlifting exercises, and therefore increases overall stability. The variability is the point, like balance training. The body a complex and dynamic system and this is meant to challenge it.
