I seem to recall that North had a similar problem a few years ago. They'd lead comfortably and then cough it up in a quarter and a bit to lose narrowly. Scott's excuse at the time was that the players weren't following his instructions.

The best one was against Adelaide at Etihad when Kerridge (!) kicked 6. 5 goals up with about 5 minutes left from memory and Crows won by a point.
