It is mostly just a quirk of how the qualifiers are set up.

Mexico, USA
Japan, South Korea, Australia

Those countries should be qualifying every single time in the current format. USA screwed the pooch once out of 6.

Africa is always a raffle and Europe is competitive because a Croatia or a Denmark or whoever have enough talent to knock one of the big nations off every now and then
