only the judge (mark evans) that didn't even give crouch a vote that was surely wrong

Given that 44 votes went to O'Meara, the highest score possible from there was 37 votes. Given that Evans didn't give Crouch any votes at all, that means he only got one vote below the top possible score. So all but one judge (excluding Evans) voted O'Meara and Crouch as their top two.

One judge must have put him third, presumably with Wines at #2.

Reilly was definitely nominated - it was against Melbourne in 2004, in Craig's first game as caretaker coach, just after Ayres walked the plank.
