I feel sorry for people who actually paid to see him in concert - he is clearly not in great health and is struggling through a 15 minute set. How would he get through a full concert?

Me too, although it may be easier in concert as he will be singing the full songs and will be able to take a bit of a breath during the verses. Here he's just about only singing the choruses, which are often the hardest, fastest, highest, loudest parts of the song and it doesn't give him much chance to catch his breath.
The start and the end were OK, but the parts in between were cringewhorthy.

It is a well known fact he lost the ability to sing a long time ago. If I remember correctly, it is health related.
