Doesn't stop the CEO taking the credit (and bonuses) for success. In cases of criminal negligence, the buck often has to stop with the boss. If it can be proven they were aware of the issues and didn't put in the measures to fix them, jail time is appropriate.Very hard to single out someone in these cases. A CEO isn't necessarily inspecting rides and writing policy. There are maintenance teams and safety teams in place.
Sure they report to Operations Managers who report to the CEO and the right systems should show sign offs on every issue and demonstrate where in the chain things break down.
There will be multiple people at fault here but wilful negligence by a single person ... very difficult to prove.
A memo to say don't use the e-stop though, who approved that? What alternatives were put in place and how was the info disseminated? That's the big issue. Bypassing a safety control ... you'd want to have a very good reason.
