people who have had their vocal chords removed don't actually 'talk through' the stoma - it is just used for breathing and coughing stuff up. Usually they block the hole when speaking.
they talk either using the esophageal voice, or some sort of prosthesis to simulate the vibration and tonal modulation usually produced by the voice box - the sounds are still largely shaped in the mouth (hence the accent persisting)
there are also other learned aspects of accent (e.g. how you use words and which syllables you tend to stress) which are unrelated to the physical mechanics of speaking
