Let's go with your questions though.

Whose responsibility to identify and remedy: the latter is key as power is in the hidden forum no? Those elected to give a damn about league health in a modicum of respect? Say LG turfing a rep every week based on whims would be sure to not irritate anyone involved at all...
Is it not reasonable to have LG or other members speak to rep and checks & balances: Internal club dynamics, if they are not made aware of an issue is their ignorance/apathy justified?
Does transparency fix any of this: yes/no. Highlights competency, highlights deficiencies, mob rules, effectively light trolling all things considered.

Essentially, style of admin matters, as autocratic has power and committee is there for the ride, democratic does not as populist diatribe demands majority rule, power is then split between admin and committee and not solo. Not necessarily an admin only blame game for issues, consistent is collection of individuals into committee with differing focus, none at all, or anything in between. In those, transparency then has committee going "we have these problems, these reps are bad" and doing nothing at all about it. Very easy then for a democratic admin to turn autocratic for the health of the overall league.

And yes, they would be just due to how they are elected, there's say a very good reason AFL house doesn't have current day players in it yet AFLPA does, any "issue" in committee will inherently feed back simply due to clubs having to have representation.

All of this is still to say it’s not my business as a rep of my club to dictate to other clubs about the reps they choose.

If it’s identified as an issue for the club and therefore the league, the admin has the ultimate responsibility to remedy it.
